The role of aspirin and clopidogrel as antithrombotic agents in atrial fibrillation

Abstract

|
Atrial fibrillation (AF) is the most common cardiac arrhythmia and an independent risk factor for stroke. The prevention of stroke using anticoagulation or antiplatelet agents in patients with AF has been a major focus in clinical practice. Warfarin has been the most commonly adopted therapeutic option for stroke prevention for several years. Antiplatelet agents, including aspirin and clopidogrel, have a limited role and an inferior therapeutic strategy in stroke prophylaxis. However, aspirin combined with clopidogrel shows modest benefits and a similar bleeding risk in patients with AF who are not eligible to receive oral anticoagulant drugs. In the early stage of acute coronary syndrome and after stenting, aspirin administration should be continued, and its combination with clopidogrel is recommended. However, the role of antiplatelet therapy for stroke prevention in patients with AF in the era of new oral anticoagulants remains unclear.
